This form should be completed and turned in to your child's teacher. The information would be used in the event of an emergency or unscheduled school closing. There is also a section that is used to give permission for your child to be videotaped, interviewed, or photographed for release within the Howard County School System. Remember if this information changes it is your responsibility to contact the school with the updates.

This form is used to give permission for your child to go to Cedar Lane school for art and music. This form is to be filled out for 3rd grade students only.

This form is used to give permission for your child to take a walk on the Fulton Campus or ride the campus bus.

This form is used to join the Fulton Elementary School PTA.

Bullying Prevention

The HCPSS is committed to providing a safe and nurturing environment for all students. Preventing bullying is an important part of this goal. Our schools provide a variety of initiatives to teach students, families and staff about bully prevention. Through character education programs, such as Positive Behavioral Interventions and Supports (PBIS) and Developmental Assets, students learn conflict resolution and assertiveness skills while further developing empathy and respect. Professional development for staff and information provided for families also assist us in our efforts to keep students safe from bullying and harassment. If you believe your child is being bullied, please refer to the Student/Parent Handbook . In addition, please be sure to complete a Bullying, Harassment or Intimidation on our reporting page. By working together, we can prevent bullying.
