***Note: Any child entering kindergarten for the 2011-2012
school year must turn 5 by September 1, 2011.***
The following is required to register your child:
Student must be registered by his/her custodial parent (biological, adoptive, or legal guardian).
The following information is required at the time of registration:
- Parent must produce a picture I.D. – driver’s license, military I.D. or passport.
- Signed original current lease or deed with your address, valid dates, signatures.
- Current utility bill with name and address (non-cellular telephone, gas & electric, or cable bill).
- Student’s proof of birth (birth certificate, baptismal certificate, hospital certificate, passport, military identification).
- Student’s current proof of immunizations.
- Student’s most recent grade report – transcripts, report card from last school attended at time of withdrawal.
- If the student is receiving or has received Special Education, 504, or related services, please provide current documentation i.e. Individual Education Plan (IEP, 504, psychological report).
- Custodial parent must submit proof of custody, i.e. divorce settlement papers, court papers, or adoption papers.
STUDENT EMERGENCY CARD:
To update your Student Emergency Card, please click here. Simply complete and print the updated form and send it to school with your child. It is important that the school has your most recent information in case of emergencies.
